Skilled Nursing
The professionals at Evangeline Oaks Guest House Nursing Home & Rehabilitation Services (EOGH-Rehab) have been providing excellent, personalized skilled nursing care for local families and their loved ones for more than forty years. We offer a warm and inviting home-like environment for those who are in need of a short-term rehabilitation stay return to home or for a long-term stay.
We provide nursing and rehabilitation services through a professional, caring staff of registered nurses, licensed practical nurses, and certified nursing assistants. The team is supported by physicians, an on-site speech therapist, an occupational therapist, a physical therapist, and social services.
Skilled Nursing Services include:
- Weekly visits by the medical director. The medical director makes weekly on-site visits and is available to you for consultations as needed. Each resident may choose the medical director or may choose his or her own personal physician for their medical needs.
- Weekly visits by a nurse practitioner. In addition to the weekly physician visits, the Nurse Practitioner is also available on-site weekly and as needed for consultations.
- Monthly pharmacy review of resident’s medication. The pharmacist works closely with the chosen physician and Nurse Practitioner to ensure that all medication is appropriate.
- Monthly evaluation by licensed dietitian and nutrition services. A licensed dietitian approves all menus and monitors each resident’s individual dietary needs. We understand the importance of nutrition for overall health and we are committed to providing nutritious delicious meals to meet each resident’s nutritional needs.
- Monthly visits and care as needed by a podiatrist.
- Monthly visits by our dental director. Our on-site dental services do monthly evaluations to meet the needs of the resident. If further dental services are needed, we have a dental office in close proximity to the facility or we can make the arrangements for an appointment with the dentist of your choice.
- Monthly behavior specialist review and consult as needed

- Environmental Services – this team takes care of all housekeeping, laundry, and grounds maintenance to help provide a safe, comfortable, and home-like environment for our residents.
